SUBJECT:    CHECKLIST FOR ELIGIBILITY AND PLACEMENT

 

To apply for a Coop or an Internship position you must have the following completed:

            _____  C Ave., good attendance,  no major discipline problems

           

            _____  Application (Must be typed) www.paris95.k12.il.us/phs/guidance

                         Forms copy as a word document

            _____  Resume

 

            _____  Transcript (we will obtain after you turn in application)

 

            _____  You should meet with Mr. Thomas after you apply

 

To be placed and have your schedule changed you must complete the following:

 

            _____  Training Agreement (signed by the student, parent/guardian,

employer/business partner, and Mr. Thomas)

 

            _____  Absence Reporting Procedures (signed by the student and parent/guardian)

 

After all of the above items are complete, then I will authorize the guidance department to change your schedule.

 

Work reports must be turned in—this is required.

 

Internship Students must turn in a journal at the end of each quarter—this is required.

 

All students must participate in employer/business partner appreciation luncheon at the end of the school year (usually late April)—no exceptions.

 

Students must follow all rules and guidelines of the program or he/she will be removed with a failing grade.
